     SERVICES PROVIDED:  Comprehensive health services including
     treatment and preventive care.  Services offered include ambulatory
     medical care, optometry, physical therapy, audiology, dental care, public
     health nursing, mental health/social services, WIC PROGRAM (Women,
     Infants and Children), nutrition counseling,
     maternal child health, home health, and health education services. 
     Specialized clinics held regularly include well-child services (Tuesday and
     alternating weekly field clinics in White Swan and Wapato), diabetic clinic
     (Wednesday), prenatal clinic (Thursday), and women's health care
     (Friday), in addition to smoking-cessation clinic every other month
     (advance arrangements necessary).  Health services not available through
     the clinic may be provided through contracts with local providers.  Prior
     approval is required for contracted services.

     ELIGIBILITY REQUIREMENTS:  Services available to any person of Indian
     descent from a federally-recognized Indian tribe.

     AREA SERVED:  Yakama Indian Reservation and immediate surrounding
     area.

     FEES:  None to individual Indian persons.  Medical insurance, Medicaid
     and other third-party resources will be billed on behalf of patient, if
     available.

     HOURS:  Service unit hours are 8:00 a.m.- 5:00 p.m. Monday through
     Friday.  Closed Thursdays 8:00 a.m.- 12:30 p.m. 

     REFERRAL PROCEDURE:  Call or walk in.  Appointments encouraged.
