Request New SKIES User

Requirements:

  • User's agency or organization has valid WorkSource MOU, OR
  • Has a contract with WDC or ESD for WIA, with access, disclosure and security written into a SKIES Data Sharing Contract; AND
  • User's agency or organization has signed SKIES Data Sharing Agreement with ESD; AND
  • User has completed SKIES training; AND
  • User has signed SKIES Notice of Non-disclosure.

Procedure:

NOTE: A new user's account can never be activated until the paper forms are faxed or delivered to the System Administrators at the WDC. Three (3) forms are required: User Activation Request, Training Verification, Non-Disclosure -- all appropriately signed.


Request SKIES User to be De-activated

NOTE: A user whose employment is terminated must be deactivated IMMEDIATELY (within no more than 24 hours)!

Do not delay.

Procedure:

  • Access Approver completes and signs De-Activate form. Use   SKIES User Deactivation Request, and print it.
  • User's supervisor signs De-Activate form.
  • Signed De-Activate form is sent to South Central WDC. (FAX PREFERRED)
