Beginning January 1, 2021, proof of identification and relationship will be required to obtain a birth or death certificate. Please bring proof of identity when ordering birth or death certificates.   • Cash: Only accepted at the counter during our normal business hours.

Funeral Homes

Ordering Stillbirth Records

As of October 1, 2022, Stillbirth Certificates are available. These certificates formally acknowledge the birth of a child who passed away after 20 weeks of gestation and resemble birth certificates. They include information about the child and their parent(s) and can only be requested by the person who gave birth.

 

Ordering Fetal Death Certificates

A fetal death occurs when a fetus of 20 or more weeks gestation is not born alive. The Washington State Department of Health collects information on these events, sometimes referred to as stillbirths or miscarriages. Fetal death data includes details like cause of death, mother's age, and fetal weight. Only annual files are available.

Eligible applicants include parents, siblings, grandparents, legal representatives, funeral directors (within 12 months of the death), or government agencies/courts for official duties.

 

Pre-Adoption Records Request

The Yakima Health District does not issue pre-adoption birth certificates. To request one, contact the State Department of Health Vital Statistics at 360-236-4300 (option 3).

 

Parentage

As of January 1, 2019, the Uniform Parentage Act is in effect. The Acknowledgment of Parentage (AOP) replaces the Paternity Acknowledgment form for unmarried parents. Denial of Parentage (DOP) is a separate form. Both can be witnessed or notarized and must be submitted to the Department of Health (P.O. Box 9709 Olympia, WA 98507) with an $18 fee. Spelling and Name Corrections

Spelling corrections or name changes for children under one require an Affidavit for Correction. For individuals over one, a Court Ordered Name Change is needed. All forms must be sent to the Washington Department of Health. The Yakima Health District does not process corrections; we print certificates as issued by the state.
